•The provision of this data is necessary for us to understand your current health, identify any Red Flags and enable us to work with you using our protocols, in case of failure to provide such data you may not be able to use Muscle Therapies’ services;

•We provide you the opportunity to give us any additional history information as it comes up. Whilst the provision of this data is completely voluntary any information you give us will aid our ability to treat you. How is your data stored?

•Medical history and personal data is removed from the website as soon as you submit your History Form to us. It is then encrypted with a password on an external storage drive which is also password protected. This is then keep in a locked safe when unattended.  

•Our email system will retain your email address on the computer. This computer is under password protection whenever it is not in use. 

 

Third Party Disclosure

We do not sell, trade, or otherwise transfer Personally Identifiable Information. We do however pass information anonymous regarding your IP address through our cookies to Facebook and Google for website performance analytics and remarking.

Occasionally, at our discretion, we may include or offer third party products or services on our website. These third party sites have separate and independent privacy policies. We therefore have no responsibility or liability for the content and activities of these linked sites. •Right to rectification which is the right to obtain from Muscle Therapies without undue delay the rectification of inaccurate personal data that concerns you.

•Right to erasure which is the right to obtain from Muscle Therapies the erasure of personal data that concerns you without undue delay and the Muscle Therapies shall have the obligation to erase this personal data without undue delay where certain conditions under the GDPR are met. This right may be extended to the “right to be forgotten” which will apply if Muscle Therapies have made some of your personal data public.

•Right to restriction of processing which is the right to obtain from Muscle Therapies restriction of processing where certain conditions are met.
